1. Read the DS-11 form in its entirety before filling out.

2. Select BOOK, CARD, OR BOTH.

3. Give the Commercial Name you desire to have on your National Citizenship Identity document(s)

4. Give DOB, CITY AND STATE, SSN, EMAIL ADDRESS, CONTACT NUMBER AND MAILING ADDRESS. *Write your City, skip the STATE-ABBREVIATE box and write your “State” as your “Country, DO NOT ABBREVIATE.

5. In box 9, enter any Assumed Name or names used in the past. (Examples: Birth name, etc.)

6. DO NOT FILL OUT THE LOWER PORTION OF PAGE 1; DO NOT SIGN UNTIL IM THE PRESENCE OF AN AUTHORIZED OFFICAL.

2. BOX 13: *FOR THOSE SEEKING RELIGIOUS AFFIRMATION, Your occupation should be MINISTER/PRIEST (or whichever title according to your Religious Organization aligns with the Status of your Organization’s Clergy)

3. Box 14: If you’re obligated to a Business as employee or enrollee, this should be your Occupation. (Example: A Minister is employed by its Religious Organization, a Moorish American Priest is employed by the Moorish Science Temple of America.)

My Nationality and Identification Card for the Moorish Science Temple of America is valid for ID in the US, pursuant to: US DEPARTMENT OF JUSTICE CRIMINAL RESOURCE MANUAL 1508 TITLE 18 SECTION 1028.

• MAKE TWO (2) COPIES OF IDENTIFICATION AND AN EXTRA COPY OF YOUR DECLARATION IF APPLYING AT THE POST OFFICE.

*THE US DEPARTMENT OF STATE WILL CONTACT YOU REQUIRING YOU TO PRESENT THIS INFORMATION FOR THEM TO PROCESS.

1. US Citizenship relates to Federal District Government Administration of one’s Personal Estate Identity, not to one’s National Republic Government Administration of one’s Real Estate Identity.
